Mackle’s Diverticulitis corresponds to a subtype of the small intestine. Endometriosis is a gynecological disease, caused by the presence of functional endometrial tissue, out of the uterine cavity. The objective is to show this case because it is infrequent to coincide these two entities in a complicated way. A 35-year-old female patient goes to the hospital complaining of pain, abdominal distension and no expulsion of feces or gases. She was operated and complicated Mackle’s diverticulitis, intestinal perforation and a colon recto sigmoidal lesion were found, in the serviette ring with posterior histopathology confirmation of intestinal endometriosis. Endometriosis is a disease that impacts the life quality of the patients who suffer from it, the colorectal compromise is not frequent and when it appears, it is many times invalidating. In the context of benign diseases, there is a tendency towards conservative surgical techniques, but in this case a radical surgery is performed because of the way of its emergency presentation.
